Call for Papers
2011 Annual Meeting
July 26 - 30, 2011
Please use this form to submit a talk or to suggest a discussion panel. Please complete all of the fields and then click the "submit" button at the bottom of the form. We would appreciate receiving your submission no later than May 31, 2011 in order to meet the requirements for our CME accreditation. However, sooner would be better!
Be sure to include a short abstract in addition to the title of your talk (2-3 sentences). Also, due to new CME requirements, we ask that you also provide a one-sentence that states the "educational need" or the "gap in knowledge" your lecture addresses.
If there is a day of the meeting you cannot speak (such as arriving late or leaving early), please indicate that so we can try to accomodate your schedeule.As in the past, we ask that the lecture portion of your presentation be limted to eight minutes, with another seven minutes reserved for group discussion and questions.
